Understanding Sciatica

Sciatica pain can range from a mild ache to a sharp, burning sensation, and in some cases, it may feel like a jolt or electric shock. This condition is often caused by age-related changes in the spine, including herniated disks or bone spurs. Sciatica specifically affects the sciatic nerve, the largest nerve in the body, which runs from the lower back down the back of each leg.

Sciatica Treatment

Sciatica refers to pain that radiates down the leg from the lower back. This pain may travel along the back, outside, or front of the leg and is often sudden in onset, commonly triggered by activities like heavy lifting. However, it can also develop gradually. The pain is frequently described as shooting and typically affects only one side of the body, though certain causes may result in bilateral symptoms. Lower back pain may accompany sciatica, along with weakness or numbness in the affected leg and foot.

Common Causes of Sciatica

  • Spinal Disc Herniation: Accounts for about 90% of cases, with a disc pressing on the lumbar or sacral nerve roots.
  • Other Causes:
    • Spondylolisthesis
    • Spinal Stenosis
    • Piriformis Syndrome
    • Pelvic Tumors
    • Pregnancy

Diagnosis

  • Straight-Leg-Raising Test: Positive when pain shoots below the knee as the leg is raised while lying down.
  • Imaging: Generally not required but may be used in cases involving:
    • Bowel or bladder dysfunction
    • Significant loss of sensation or strength
    • Persistent or severe symptoms
    • Suspicion of tumor or infection

Differential Diagnosis

Conditions such as hip disease or early shingles (before the rash appears) may present with symptoms similar to sciatica. Proper evaluation is essential for accurate diagnosis and effective treatment.
